Job Overview
:
The Case Manager provides case management services in conjunction with recovery support services. The Case Manager is responsible for effectively guiding patients with substance-use disorders through the community and social service systems needed to initiate and maintain recovery. The Case Manager will screen the individual for social determinants of health, determine the most appropriate services to address their needs, educate the patient on their options, and remove any barriers associated with access to care.
The Case Manager will manage follow-up appointments to coordinate continuation of care and linkages to community resources. The Case Manager will also work alongside the Recovery Specialist team to support the patient and ensure the highest quality of care.
Qualifications
:
Required:
- Bachelor's Degree
- Minimum 2 years Case Management experience
- Must possess the knowledge, skills, and experience necessary to completely perform case management
- Demonstrate a commitment to diversity, equity, and inclusion through continuous development/training, modeling inclusive behaviors, and proactively managing bias
Preferred:
- CADC certification

Essential Functions
:
- Complete necessary documentation in the electronic medical record
- Coordinate care with community-based organizations to provide services to address individuals' needs
- Develop a person-centered Integrated Case Plan that identifies the individual's priority of needs, strengths, risk factors, and motivation to use the most effective strategies and connect the patient with resources that best support their recovery goals
- Ensure a seamless continuum of care to provide the patient with the best opportunity to maintain recovery and fulfill needs
- Linkage and advocacy with and to appropriate care and community resources, including services for specialized needs, such as HIV/AIDS, behavioral health, chronic and acute health problems, and criminal justice system involvement
- Promote the individuals' recovery to their fullest potential by facilitating assessment of skills, including literacy and educational opportunities
- GED, certification programs, job readiness skills, vocational training, employment placement and retention, and career development
- Use of motivational interviewing and engagement skills to motivate individuals to voluntarily engage in services that would assist in maintaining a healthy lifestyle
- Provide a comprehensive case management assessment that addresses social determinants of health such as housing, finances, transportation, legal services, vocational, employment, healthcare, and family strengths/needs
- Provide relapse prevention education
- Prepare all reports required by the funders
- Perform all other duties as assigned
